BP and Heart Rate Not Monitored, Medication Administered Without Physician Order

University Behavioral Health of Denton was cited by the Centers for Medicare and Medicaid Services (CMS) for failing to supervise and evaluate the nursing services provided to one patient. No documentation was found to establish that patient’s vital signs (blood pressure and heart rate) were re-evaluated by a hospital nurse or that the nurse provided intervention, even though the patient exhibited symptoms such as drowsiness and vomiting.

CMS also cited the facility for failing to administer medications per protocol, after a nurse administered Ibuprofen without a physician’s order.
