Children's residential unit beds were missing bed linens

The Texas Department of Family and Protective Services (DFPS) cited Meridell Achievement Center for failing to comply with the requirement to “provide each child with pillow/bed linens appropriate for temperature.” DFPS wrote:
Some beds in the units did not have bed linens. Also, the calming rooms should be supplied with protective covers on the pillows or some way to disinfect the pillows used in the calming rooms.
The facility was also cited on the same day for violating the standards for bathrooms. DFPS noted, “A couple of toilets in the female units did not flush properly and appeared to be ready to overflow.” These deficiencies were uncorrected at the time of inspection, and were not verified as corrected until four months later in September 2014.
