Deficiencies in Incident Reporting Inhibit Care Improvement Tracking

The Florida Agency for Health Care Administration (AHCA) found that Manatee Palms Youth Services violated its policies and procedures for incident reporting. An incident report was not filled out completely for a patient restraint incident in which a staff member bent the patient’s hand backwards to prevent him from punching another staff member. In another incident, the facility failed to submit a report after a patient acted out by causing property destruction and spitting on and throwing punches at staff, among other defiant activities. AHCA found that the failure to implement the incident reporting policy “does not ensure all incidents are available for tracking and trending to improve patient care.” As a result, the facility was required to submit a plan of correction to address its cited deficiencies.
