Facility fails to conduct employee training or performance improvement plan, despite documented problems

North Springs Behavioral Healthcare was cited by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) for deficiencies related to policies and procedures around “supervision, frequency, and documentation.” DBHDS found that:
“Supervision notes document issues with an employee’s performance; however, there is no documentation of improvement plan or additional training to be implemented.”
The provider responded that the Unit Coordinator met with the employee about performance improvement and re-training was completed with Unit Coordinators on how to write performance improvement plans. However, the provider did not indicate if additional training and supervision was provided to the employee.
