First Home Care cited for not making reasonable efforts to obtain prior assessments of intellectually disabled patient

First Home Care was cited by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) for deficiencies related to efforts in obtaining prior patient assessments. DBHDS noted that a 15-year old patient was admitted to the program and had received services from other providers prior to coming to First Home Care. However, First Home Care did not obtain information releases or other previous documents as a part of the patient’s initial and ongoing assessment. The patient had a psychological evaluation 8 months after being placed into the program that revealed an intellectual disability. DBHDS noted that “this information may have been available from the school system at the onset of IIH service delivery.” DBHDS commented that “the provision of case management and ongoing service coordination for the client and family is a component of IIH services.” The provider submitted a plan of correction to address this cited deficiency.
