No evidence that seven out of twelve patients consented to treatment

The Centers for Medicare and Medicaid Services (CMS) cited The Bridgeway for failing to comply with federal requirements to obtain patients’ consent for admission and treatment. CMS found that seven out of twelve patients had no signed consent for treatment forms in their medical records. CMS wrote:
Failure to obtain signed consents for care did not afford the patient or their representative the opportunity to make informed decisions regarding his or her care.
