Restraint and Seclusion Initiated Without Documented Physican Order

The Centers for Medicare and Medicaid Services (CMS) cited Laurel Ridge Treatment Center for deficiencies related to upholding patient rights following an unannounced complaint investigation, finding the facility failed to use patient restraint and/or seclusion in accordance with hospital policy and state law.

CMS found that Laurel Ridge had no documentation of a telephonic physician’s order for an episode of restraint and seclusion that was initiated by a nurse.
