"Time Toileting" Plan Disregarded, Boy Experiences 144 Incidents of Bladder & Bowel Incontinence

Parents brought concerns to surveyors related to their child’s safety, grooming, and finding him in bed incontinent of bowel and bladder. When investigating this violation, surveyors observed the patient being placed in front of a television to help keep calm. After 2 hours and 43 minutes in front of the television, staff did not attempt to toilet the child even though his care plan required him to be “time toileted” every two hours. Review of the child’s medical record revealed the boy had experienced at least 124 incidents of urinary incontinence and 22 of bowel incontinence in the month of August 2011.
