First Home Care Roanoke Branch was cited by the Virginia Department of Social Services (DSS) during an unannounced monitoring inspection for violating standards around staff qualifications, intake assessments, treatment plans, face-to-face contact with foster children, and adequate treatment services.
Virginia DSS found that an employee’s personnel record did not contain adequate information that the employee had met the required qualifications for the position he/she held. First Home Care has been cited for similar deficiencies related to staff qualifications In September 2013 and September 2012. Also, in another personnel record, DSS found that the central registry background check was more than 30 days late.
DSS also cited the program for not securing prior documentation and assessments from other treatment providers and previous placements for at least two individuals. Similar to the above citations on staff credential requirements, First Home Care was previously cited by the Virginia Department of Behavioral Health and Developmental Services because the provider did not make reasonable efforts to obtain prior patient information. Lastly, DSS found service breakdowns at the program after one case file showed that an individual had not received the required twice monthly face-to-face meetings and another individual, a 3 year old child, had not received adequate treatment services in a home setting.
The program was required to submit a detailed plan of correction to address the cited deficiencies.
